With regard to Tom Kuegler, I think it's possible that like many sales people, he isn't too insightful. There's also an element of vanity. Many people who achieve some success tend to have people socially beneath them be super polite to them. People on the same social strate or higher don't fear being penalized in some way, so they don't have to be polite to someone they don't like or approve of.
